The form also includes fields for property and inspection information such as PHA, family identifier, housing type, year constructed, number of bedrooms, address, owner, inspector, type and date of inspection, and whether children under age 6 reside or are expected to reside in the unit.<br /><br />At the end, the checklist states that it is a voluntary form intended to support HUD oversight and improve housing quality under NSPIRE regulations.
